Took me 10 rolls to realize my lens cap was still on for half my shots
I was shooting a full day at the Brooklyn Botanical Garden last spring and kept wondering why the viewfinder looked so dim. Got the film back from the lab and 12 out of 36 shots were just completely black frames because I never fully twisted the lens cap off. Has anyone else had a dumb gear mistake that took way too long to figure out?

Forgot to take the lens cap off for a whole roll at my nieces wedding once.

Not to be that person, but you probably mean you shot a whole roll with the lens cap on. Unless you were actually removing the lens cap for each shot, which would be a different kind of disaster.
