Three years into this shop job and I still can't get barrel adjusters to stop slipping

Been fighting a Trek road bike since last Tuesday where the rear derailleur cable keeps going slack after 10 miles. I swapped the cable three times and even put a fresh housing on it but that little plastic wedge just won't hold. Anyone else had to switch to those metal locking adjusters or am I missing something obvious?

Are you using a standard plastic ferrule or one of those little metal end caps on the cable housing where it goes into the adjuster? I had a similar headache a while back and found out the plastic ferrule was slightly too big and it wouldn't let the wedge grip the bare cable right. Also make sure you're trimming the housing perfectly square, even a tiny angle can let the cable shift under tension.
