Hit 500 e-signature audits this month and 30% had a missing witness field

We do document reviews for small firms and I track every e-signature file we touch. This month crossed 500 total audits. I did a quick breakdown and 150 of them were missing the witness signature block entirely. That means those agreements could get challenged in court in most states. Clients assume the platform validates everything, but it just collects signatures. We caught it before any of them filed, but I'm wondering if anyone else has seen this same ratio in their own workflow? How do you handle the gap between what e-sign tools promise and what they actually check?
